MEMBERSHIP SERVICES
TIER I - PRIMARY CARE MEMBERSHIP

Routine follow up visits and preventative care.

Management of chronic illnesses within the scope of primary care such as diabetes, hypertension, and asthma.

Treatment of acute illnesses.

Basic mental healthcare services within the scope of primary care, addressing common conditions such as anxiety and depression.

Women’s healthcare.

And all other primary care concerns for all ages!
TIER I MONTHLY PRICING
Adults 18-44 years old- $60/month
Adults 45-64 years old- $90/month
Adults 65 years or older- $120/month
Children under 18- $25/month with an adult membership
TIER II - FUNCTIONAL SPECIALIZED CARE MEMBERSHIP

Functional health assessment and treatment plan.

Hormone replacement therapy (excluding bioidentical pellet therapy, which does not require membership)

Personalized management of conditions such as chronic EBV, autoimmune disease, mold toxicity, metabolic disorders and weight loss, leaky gut, and more.
